Crossroads Hospice & Palliative Care of Memphis

Crossroads is an accredited hospice and palliative care provider serving the Memphis, TN metro area. We serve patients in Shelby, Fayette, and Tipton Counties. Our ultimate goal is to help our patients and their families in anyway possible. This is accomplished by celebrating life and enjoying what time is left.

Crossroads Hospice Palliative Care Memphis provides hospice and palliative care to patients in Shelby, Fayette. and Tipton Counties. We travel to patients at homes and in nursing homes as well as assisted living facilities.

We have several programs designed to help patients appreciate their lives, including Gifts of a Day and Life Journals.

We also have volunteers who provide monthly visits to patients and their families for extra socialization, companionship and emotional support.  Volunteers also help with monthly blessings, cards to patients, baking, sewing, and crafts.
